For those benefiting from SSK health benefits under social security agreements with foreign countries, a photocopy of the "Health Assistance Certificate According to the Social Security Agreement," issued and approved by the Provincial Social Security Directorates, will be required for processing. This certificate, which specifies the validity period, covers treatment services provided between the specified dates.

Applications must be made with a national identity card bearing a Turkish Republic identity number.
