HOW ARE ADMISSION, REFERRAL, AND DISCHARGE PROCEDURES CARRIED OUT?

If a patient is admitted to the inpatient clinic after being discharged from the outpatient clinic, the physician will register the patient through the information system. The patient goes to the designated clinic with their ID and a photocopy of the ID and reports to the relevant clinic staff. Every patient admitted to our hospital is given an identification wristband; please do not remove it while you are hospitalized.

**Operation of our Inpatient Clinics:**
• Patients admitted to our hospital may bring their own personal items such as slippers, pajamas, a cup, and optionally forks, spoons, plates, napkins, toothbrushes, etc.
• All patient test results are monitored and archived in their file by our hospital staff. Patient relatives do not need to follow up on the results. Furthermore, original test and examination results are not given to the patient or their relatives. • If the patient has no relatives, their valuables are taken into custody by the clinic and the necessary administrative procedures are carried out. The return of the items to the patient and/or their relatives is documented.

Referral Procedures
If your patient needs to be referred to another institution for any reason, the referral form is prepared by the physician.
For advanced treatment and treatment that cannot be performed in our hospital, the patient is referred according to the treatment. The referral process is provided by 112 ambulance or, if deemed necessary by the physician, by the patient's own means of leaving the hospital. Patient Discharge Procedures
The patient and/or their relative are informed when the decision for discharge is made. If the patient is discharged, a copy of the epicrisis report, which is a summary of all interventions, is given to the patient. Please remember to take your epicrisis document (patient discharge summary) when you are discharged. The patient and/or their relative are given information about their diet, exercise, education, etc. before leaving the hospital. After the discharge procedures, they are informed that the treatment is finished. The identification wristband is removed upon discharge. For those without social security and those who are not injured, the discharge procedures continue until the payment is made.
